A Prospective Cohort Study of PD-1 Monoclonal Antibody Combined With Ipilimumab N01 and Chemotherapy in Perioperative Treatment of Resectable Non-small Cell Lung Cancer

A Prospective Cohort Study of Perioperative PD-1 Monoclonal Antibody Combined With Ipilimumab N01 and Chemotherapy in Patients With Resectable Non-Small Cell Lung Cancer

Source: ClinicalTrials.gov · Last updated 5 Nov 2026

Brief summary (registry, in English)

This multicenter, prospective, randomized controlled study aims to evaluate the efficacy and safety of different durations of neoadjuvant treatment with a PD-1 inhibitor, ipilimumab, and chemotherapy in patients with resectable non-small cell lung cancer (NSCLC). A total of 90 treatment-naïve patients with clinical stage IIA (primary tumor >4 cm) to stage IIIB (limited to resectable N2 disease) NSCLC will be enrolled and randomly assigned in a 1:1 ratio to Cohort A (n=45) or Cohort B (n=45). Patients in Cohort A will receive two cycles of a PD-1 inhibitor plus chemotherapy and one cycle of ipilimumab, whereas patients in Cohort B will receive four cycles of a PD-1 inhibitor plus chemotherapy and two cycles of ipilimumab. The PD-1 inhibitor will be administered intravenously at 200 mg every three weeks, and ipilimumab at 1 mg/kg every six weeks. Chemotherapy regimens will be selected according to histological subtype: pemetrexed plus carboplatin or cisplatin for non-squamous NSCLC, and nab-paclitaxel plus carboplatin or cisplatin for squamous NSCLC. Following neoadjuvant treatment, eligible patients will undergo curative-intent surgery within 22-42 days after the last treatment dose. Investigator-selected adjuvant therapy will begin 29-42 days after surgery. Tumor assessments will be performed according to RECIST version 1.1, with postoperative follow-up for up to five years. The primary endpoint is the pathological complete response (pCR) rate, with treatment safety also being evaluated.
